Abstract
ABSTRACT Background: The administration of intrathecal opioids as an adjuvant to spinal anesthesia in patients undergoing Caesarean Section (CS) significantly improves postoperative analgesia quality but potentially elevates the incidence risk of Postoperative Nausea and Vomiting (PONV). This clinical study specifically aimed to evaluate and compare the explicit incidence rate of PONV in elective CS patients undergoing spinal anesthesia with and without additional intrathecal opioids. Methods: This prospective analytical observational clinical study was systematically conducted from May to July 2026 at two distinct general hospitals located in Lhokseumawe. A total sample size of 70 eligible respondents was meticulously recruited using a consecutive sampling method. The participants were equally divided into two treatment arms: an opioid-adjuvant group utilizing fentanyl (35 patients) and a non-opioid control group (35 patients). Comprehensive bivariate statistical data analysis was subsequently performed using the Chi-square test criteria. Results: The Chi-square test evaluation demonstrated a statistically highly significant difference in PONV development between the observed groups (p = 0.001). The absolute incidence of PONV within the intrathecal opioidadjuvant cohort reached 60.0% (21 respondents), whereas the non-opioid baseline group demonstrated a significantly lower manifestation rate at only 20.0% (7 respondents), indicating a distinct clinical discrepancy. Conclusion: There is a statistically significant difference in the explicit clinical incidence of PONV between elective CS patients undergoing spinal anesthesia utilizing additional fentanyl opioids compared to those without opioid adjuncts. The inclusion of intrathecal fentanyl significantly enhances the relative risk occurrence of early PONV. Keywords: Caesarean Section, Fentanyl, Opioid, PONV, Spinal Anesthesia. ii ABSTRAK Latar Belakang: Penggunaan opioid intratekal sebagai ajuvan anestesi spinal pada pasien yang menjalani operasi Sectio Caesarea (SC) secara signifikan meningkatkan kualitas analgesia pascaoperasi, namun berpotensi meningkatkan risiko kejadian Postoperative Nausea and Vomiting (PONV). Penelitian klinis ini bertujuan untuk mengevaluasi serta membandingkan angka kejadian PONV secara spesifik pada pasien SC elektif yang menjalani anestesi spinal dengan dan tanpa tambahan opioid intrathekal. Metode: Penelitian observasional analitik dengan desain prospektif ini dilaksanakan secara sistematis dari bulan Mei hingga Juli 2026 di rumah sakit di Kota Lhokseumawe. Total sampel sebanyak 70 responden yang memenuhi kriteria dipilih menggunakan metode consecutive sampling. Seluruh partisipan dibagi rata ke dalam dua kelompok intervensi, yaitu kelompok dengan tambahan ajudan opioid fentanyl (35 pasien) dan kelompok kontrol tanpa tambahan opioid (35 pasien). Analisis data bivariat secara menyeluruh dilakukan menggunakan uji statistik Chi-square. Hasil: Uji Chi-square menunjukkan adanya perbedaan yang sangat signifikan secara statistik pada manifestasi PONV antara kedua kelompok (p = 0,001). Angka kejadian PONV pada kelompok dengan tambahan opioid intrathekal mencapai 60,0% (21 responden), sedangkan pada kelompok kontrol tanpa opioid menunjukkan angka yang jauh lebih rendah, yaitu hanya sebesar 20,0% (7 responden). Kesimpulan: Terdapat perbedaan yang signifikan secara statistik pada kejadian klinis PONV antara pasien SC elektif yang menjalani anestesi spinal dengan tambahan opioid fentanyl dibandingkan tanpa ajudan opioid. Penambahan fentanyl intrathekal terbukti meningkatkan risiko terjadinya PONV pada periode awal. Kata Kunci: Anestesi Spinal, Fentanyl, Opioid, PONV, Sectio Caesarea.
